Kerwin Medical Center is excited to offer LEQEMBI infusions onsite at our clinic in the Jackson building of Dallas Presbyterian Hospital!

  • WHAT PATIENTS ARE APPROPRIATE FOR LEQEMBI?

    Patients must be amyloid positive - shown through amyloid PET scan, CSF results, or blood test results.



  • WHAT A PATIENT CAN EXPECT FROM THEIR FIRST APPOINTMENT

    At the consultation appointment:

    • Dr. Kerwin will evaluate candidacy for LEQEMBI and review potential risks and benefits.
    • Dr. Kerwin will order any additional testing (for example, PET, genotyping, or neuropsychological evaluation) required to evaluate candidacy and will review results with patients once they are received.
    • Infusion and safety MRI schedule will be reviewed with the patient and tentatively scheduled.
    • KMC will submit an enrollment request for Eisai's Patient Support Program, which provides benefits verification, prior authorization services, and evaluates eligibility for financial assistance.
    • For patients with Medicare, Kerwin Medical Center will submit patient information to the CMS database as required for Medicare coverage of LEQEMBI. 
  • CLINICAL DOCUMENTATION TO SEND WITH A REFERRAL


    Clinical documentation required with a referral for LEQEMBI infusions:

    • Documentation of diagnosis of Mild Cognitive Impairment or Alzheimer's Disease
    • Brain MRI without contrast from the past 12 months
    • Lab results from the past 6 months: CMP, CBC, coagulation labs

    If available, please also include:

    • Documentation of amyloid status (Amyloid pet, CSF, or blood test results)
    • Results of ApoE4 genotyping
    • Neuropsychological evaluation(s)
  • IS GENETIC TESTING REQUIRED?

    KMC does require APOE testing to rule out complications associated with specific genetic profiles. 


    Patients who haven't already completed APOE testing can order at-home APOE test kits to obtain their APOE status prior to a consultation appointment for Leqembi.

  • WHAT INSURANCE DO YOU ACCEPT?

    Kerwin Medical Center currently accepts Medicare part B, which covers 80% of costs associated with Leqembi. The patient is responsible for their 20% coinsurance.
